Your First Visit

At Ayako Physical Therapy, you will receive a personalized rehabilitation plan that accounts for the primary, secondary, and even tertiary sources of your pain to ensure your rehab is not a “quick-fix bandaid”. We pride ourselves on providing comfortable, high quality, 1-1 patient care that is unparalleled to any other.

Prior to your visit, you will be emailed New Patient Forms. Please fill these out before your first visit, or plan to arrive 15 minutes early to fill them out in our office.

During your first evaluation, Ayako Shono will ask detailed questions about your medical history and health/fitness background to create a unique, realistic plan that meets your physical therapy goals. She will conduct a gait assessment, take initial measurements, and transparently discuss her findings with you.

Depending on your needs, you may be participating in Pilates, lifting weights, or other strengthening activities as part of the first evaluation. If you have no prior experience in these activities, no need to worry! Our staff will be with you each step of the way to demonstrate, provide correction, and answer any questions you have.

• Athletic attire: shorts, t-shirts, and sports bras are all appropriate. Please be aware that if your clothing does not allow for access to the treatment site, you may be asked to change into a gown.

• Non-marking, comfortable shoes

• Pilates socks (optional)

• Driver’s License

• Insurance Card

• Physicians’ reports (X-Rays, MRIs, post-operative reports)

• Copy of New Patient Forms

• Spare pair of shorts that permit our therapist to access and treat area(s) of complaint
