Insurance Plans
Ayako Physical Therapy is an out-of-network physical therapy facility for all PPO insurance plans, and in-network for Medicare plans. Unfortunately we do not currently accept HMO insurance plans, but do have a self-pay option available. For our patients’ convenience, our staff will call your insurance company to verify your benefits and coverage. If you have any questions about physical therapy benefits, please contact us and we are happy to help you understand coverage or payment options.
Self-Pay
